David S. Kountz is Senior Vice President, Medical and Academic Affairs, Jersey Shore University Medical Center (JSUMC), and Associate Professor of Medicine, Robert Wood Johnson Medical School. Dr. Kountz assumed his new position at Jersey Shore in April. In these roles he is responsible for the continued development of the medical staff, as well as academic development on the campus, including continuing medical education, graduate medical education, and research. Previously, Dr. Kountz served on the faculty at RWJMS for 11 years in a variety of administrative roles, most recently as Acting Senior Associate Dean for Community Health.
Dr. Kountz earned his A.B. at Princeton and his medical degree from the State University of New York at Buffalo School of Medicine. He completed a residency and chief residency in Internal Medicine at Hahnemann University Hospital, Philadelphia, Pennsylvania, as well as two non-degreed fellowship programs in faculty development (Michigan State University) and executive education (Kenan-Flagler Business School at UNC-Chapel Hill). He served as Associate Professor and Associate Chief of the Section of General Internal Medicine at Temple before joining the Dean's office at Robert Wood Johnson in 1996. He has published numerous articles on hypertension and diabetes management in high-risk populations, and has been funded by the Pew Charitable Trust, CDC, and American Group Medical Association.
Dr. Kountz is a member of the Society of General Internal Medicine, a Fellow of the American College of Physicians and Treasurer of the International Society on Hypertension in Blacks. In 2002 he was elected to the Master Educator Guild of the University of Medicine and Dentistry of New Jersey.

Dr. Lawrence Blonde is Director of the Ochsner Diabetes Clinical Research Unit in the Department of Endocrinology, Diabetes, and Metabolism, as well as an Associate Internal Medicine Residency Program Director at the Ochsner Clinic Foundation in New Orleans, Louisiana.
Dr. Blonde's clinical and research activities have focused on patients with diabetes mellitus and investigations of new therapies and health care delivery systems for them. He has also published and presented information about the use of computers to enhance medical education and patient care.
Dr. Blonde is chair of the Steering Committee of the National Diabetes Education Program, a partnership of the National Institutes of Health, the Centers for Disease Control and Prevention, and more than 200 public and private organizations working to "change the way diabetes is treated."
Dr. Blonde is a member of the Board of Directors of the American Association of Clinical Endocrinologists (AACE). He serves on the Board of Directors of the Council for the Advancement of Diabetes Research and Education (CADRE), and he is a member of the National Quality Forum Adult Diabetes Care Consensus Maintenance Committee. Dr. Blonde is chair of the American Diabetes Association (ADA) Doing Better Committee and a former member of the ADA Board of Directors. He is a current member and former chair of the ADA Professional Practice Committee, which develops practice guidelines for the care of people with diabetes.
Dr. Blonde has served on the Microsoft Healthcare Users Group Board of Directors. He has also functioned as a member of the Residency Review Committee for Internal Medicine and the Transitional Review Committee of the Accreditation Council for Graduate Medical Education. He has also been a member of the Council of the Association of Program Directors in Internal Medicine, the Council of the Association of Subspecialty Professors, and the American College of Physicians-American Society of Internal Medicine Medical Informatics Subcommittee, for which he served as chair.

Anthony J. Cannon is in group practice with Diabetes & Endocrinology Associates, PA of Hamilton, New Jersey. He is President of the Thyroid Club of South Jersey and a past president of the South Jersey Medical Association.
Dr. Cannon received his medical degree from Cornell University in Ithaca, New York. He completed a residency in internal medicine at Baystate Medical Center in Massachusetts and a fellowship in endocrinology at Temple University Hospital in Philadelphia. He is board certified in internal medicine and endocrinology.

Mrs. Caputo is a nationally recognized Advanced Practice Nurse in Advanced Diabetes Management and a Certified Diabetes Educator. Until recently, she was the Director of Special Initiatives at the UMDNJ, Continuing and Outreach Education Department, and as such was responsible for new and innovative projects through this center, many of which revolved around diabetes. She was an Assistant Professor at the UMDNJ School of Health Related Professions. She earned her Masters degree from Teachers College, Columbia University in 1996, and began her PhD studies in the field of Health Sciences. Prior to joining UMDNJ, she was a Diabetes Specialist at the Visiting Nurse Association and a Clinical Coordinator for several units at an area hospital.
She was the Project Director and Co-investigator for the TRIAD (Translating Research Into Action for Diabetes) study funded by the CDC that designated UMDNJ as a translational research center for diabetes. This study brought together the largest cohort of patients with diabetes (12,500 patients) across the nation to explore the relationships between structure, organization, processes, and outcomes of care. Besides UMDNJ, there are five other sites in the nation within this study. UMDNJ contributes 1850 members to this cohort. TRIAD is now in its second phase (TRIAD II) and is moving towards interventions. Several managed care organizations in New Jersey are working with UMDNJ on this study. Ms. Caputo was also the Director of the Central Coordinating Center for the TRIAD study, and intricately involved at the Executive Planning Committee for TRIAD.
In July 2007, Dorothy left the University but continued on as a consultant to the TRIAD grant. She is now an independent consultant both in the field of diabetes and in nursing accreditation.
Beyond this, Ms. Caputo is actively involved with the New Jersey Diabetes Council and recently was appointed by the Commissioner of Health to the newly developed NJ Diabetes Advisory Council. Dorothy was also principal investigator in a pilot study of a group diabetes intervention known as the MFDI (MultiFactorial Diabetes Intervention) which has been well received by patients and physicians alike, and is looking to expand in the near future. She was also the Principal Investigator of a DHHS minority health grant to gather clinical and socio-economic data about the Asian Indian population in New Jersey.
Ms. Caputo has lectured locally and nationally on diabetes to professional and community audiences. She is a member of ADA, AADE, ANA, NJSNA, and Sigma Theta Tau. Her hobbies include gourmet cooking, needlework, gardening, community activities, and caring for her pride and joy - her four grandchildren.

Stephen N. Davis, MD, FRCP is the Mark Collie Professor of Medicine and Professor of Molecular Physiology and Biophysics at the Vanderbilt University School of Medicine in Nashville, TN. He is also Chief of the Division of Diabetes, Endocrinology and Metabolism and is the Associate Director of two NIH-funded programs at Vanderbilt: the Diabetes Research and Training Center and the General Clinical Research Center.
Dr. Davis received his medical training at London University, London, England, and completed a fellowship in diabetes at Vanderbilt in 1991. He is a Fellow of the Royal College of Physicians in England as well.
Dr. Davis' research includes studies of hypoglycemic counterregulatory dysfunction, exercise physiology, autonomic nervous system control of metabolism and new therapies for type 1 and type 2 diabetes. He has authored over 143 original articles and book chapters and is widely recognized for his contributions to the physiology of counterregulatory responses to hypoglycemia and exercise. Dr. Davis is principal investigator on three NIH studies and one VA Cooperative study and has served on NIH review study sections.
Dr. Davis has received numerous awards, including the Southern Section AFCR Young Faculty Award, the Novartis Award for Diabetes Research, and in 2001, the Fellowship of the United Kingdom Royal College of Physicians. Last year, Dr. Davis was the recipient of Vanderbilt University's Grant W. Liddle Award for Clinical Research.

Steven V. Edelman, MD, a board certified specialist in internal medicine and metabolism, diabetes, and endocrinology, is Professor of Medicine in the Division of Endocrinology and Metabolism at the University of California, San Diego (UCSD), School of Medicine. He is director of the Clinical Clerkship Program and Co-Director of the Endocrine Fellowship Training Program at UCSD and the Veterans Administration Medical Center in San Diego.
Dr. Edelman graduated from the University of California, Los Angeles (UCLA), School of Medicine, with honors. He earned his medical degree at the University of California Davis, School of Medicine, where he was valedictorian of his class, and completed his internship and residency in internal medicine at UCLA. He subsequently completed a fellowship at the Lahey Clinic in Burlington, Massachusetts; a clinical fellowship in endocrinology and metabolism at the Joslin Clinic in Boston, Massachusetts; and a research fellowship at UCSD.
Dr. Edelman is an active member of the American Diabetes Association (ADA), the Juvenile Diabetes Association, the American Association of Diabetic Educators, and The Endocrine Society. He serves as a member of the ADA Professional Practice Committee and the Self-Assessment Subcommittee of The Endocrine Society.

Martha Funnell, MS, RN, CDE is a Clinical Nurse Specialist and the Director for Administration for the Michigan Diabetes Research and Training Center (MDRTC) of the University of Michigan Medical Center. She is also an Adjunct Lecturer in the Division of Acute, Critical and Long-Term Care Programs, School of Nursing, University of Michigan. Ms. Funnell received her master's degree in nursing (medical/surgical) from the University of Michigan, and her bachelor's degree from Lenoir Rhyne College in Hickory, North Carolina.
Ms. Funnell has authored over 100 publications and made more than 160 presentations to health professionals. Her work includes participation in the development of National Standards for Diabetes Self-Management Education and Standards for Diabetes Nursing Practice. Ms. Funnell is also responsible for the development and revisions of the MDRTC Life with Diabetes print and audiovisual materials, including curricula for health professionals and patients published by the American Diabetes Association. She is an author of The Art of Empowerment: Stories and Strategies for Diabetes Educators; 101 Tips for Diabetes Education; and 101 Tips for Behavior Change all published by the American Diabetes Association.
In 1999, Ms. Funnell received the American Diabetes Association Outstanding Educator in Diabetes Award, and in 1998, she received the Distinguished Service Award from the American Association of Diabetes Educators. Ms. Funnell was President, Health Care and Education of the American Diabetes Association in 2002-2003.

Dr. Kuritzky is a family physician affiliated with the University of Florida, Department of Community Health and Family Medicine, as a Clinical Assistant Professor. He received his medical degree from the Medical College of Virginia in Richmond, and completed his internship at Miami Valley Hospital in Dayton, Ohio.
Prior to joining the faculty at the University of Florida in 1983, where he continues his academic affiliation, Dr. Kuritzky had been a member of the University of California, Los Angeles Family Medicine Clinical Faculty. His current professional society participation includes the Association of Reproductive Health Professionals, the American Society of Hypertension, the Florida Medical Association, the American Diabetes Association, and the American Academy of Family Physicians. Dr. Kuritzky has received certification as a specialist in hypertension by the American Society of Hypertension.
Dr. Kuritzky has spent the majority of his career as an educator, and has given literally hundreds of presentations, including local and national meetings of professional societies. He has also written textbook chapters and has contributed more than 50 publications to the literature, including the New England Journal of Medicine, Comprehensive Therapy, and Hospital Practice.

A Mississippi native, Scott Nelson, MD, completed his University of Mississippi Family Medicine residency in 1992. He is an active member of AMA, AAFP, and the Mississippi State Medical Association and also serves on the Board of Directors for the Mississippi Family Practice Foundation. Among his featured print publications are articles in Forbes and Glamour magazine on diabetes; Keeping Well magazine on childhood obesity; and a cover story in USA TODAY on diabetes in the Mississippi Delta. He is a past winner (2003) of the AMA Jules Bergman award. Timothy S. Reid, MD, treats patients at the Mercy Diabetes Center in Janesville, WI and serves as the Chairman, Department of Family Practice, for Mercy Health System.
Dr. Reid graduated from John Carroll University, Cleveland Ohio, where he received the American Values Scholarship - President Honors Award all four years he attended. He earned his medical degree at the Medical College of Ohio, and completed his residency in family medicine at St. Elizabeth Medical Center in Edgewood/Covingtion, KY.

Donna Rice, MBA, BSN, RN, CDE is presently a Wellness Program Manager for the Botsford General Hospital (Health Development Network) in Livonia, Michigan, and acts as a Consultant for Wellness, Diabetes Management and Care, Men's Health, Sexual Dysfunction, and Nursing Care.
Ms. Rice received her Registered Nurses Diploma from St. Anne's Hospital School of Nursing (Chicago, Illinois) in 1973, completed her Bachelor of Science in Nursing in 1976 from the Madonna University in Livonia, Michigan, and received her Masters in Business Administration from Baker College in 2004. She has also attended Triton College (Melrose Park, Illinois) and been certified in Advanced Critical Care (1973).
Donna Rice has been involved with Diabetes Management for the past 15 years. Amongst many accomplishments, she created a program focusing on men's health issues, which concentrates on erectile dysfunction and prostate disease. She has chaired a National Task force and developed a national program on erectile dysfunction for the American Association of Diabetes Educators. Finally, Ms. Rice is frequently called upon to speak on the topic of diabetes education and has authored many publications on erectile dysfunction as it relates to diabetes.

He is Associate Program Director, Endocrine Fellowship Program, Division of Endocrinology at the University of Medicine and Dentistry of New Jersey, Robert Wood Johnson Medical School. Dr. Sher is currently also Clinical Associate Professor, Department of Medicine also at UMDNJ - Robert Wood Johnson Medical School.
Dr. Sher earned his B.S. at the University of Pittsburgh and his medical degree from the Downstate Medical School in Brooklyn, NY. He completed a residency in Internal Medicine at Nassau County Medical Center in East Meadow, NY. He completed a fellowship in Endocrinology at the University of Miami School of Medicine in Miami, Florida. He served as Clinical Assistant Professor in the Department of Medicine at Downstate Medical Center in Brooklyn, NY. Dr. Sher has also served as the President and Treasurer, Medical Staff and Co-Director, Division of Endocrinology at Woodhull Medical Center, Brooklyn, NY.
He is a chairman of the Pharmacy and Therapeutics Committee and chairman of the Computer Committee at Jersey Shore University Medical Center as well as a member of the Department of Medicine Performance Improvement Committee. Dr. Sher is co-chairman of the Drug Utilization Committee and the Meridian Health Formulary Committee as well as a member of a host of professional societies, including the American Diabetes Association and the American Academy of Clinical Endocrinologists. Dr. Sher is a Fellow of the American College of Endocrinology and a Fellow of the American College of Physicians. Dr. Sher is also a Diplomat of the American Board of Forensic Medicine.
